#2 Meet director must receive: Entry Blank, Entry Fee, Membership Number or Fee, and copy of birth certificate or other proof of age, i.e. Drivers License BY Thursday, June 17, 1999; postmarked no later than June 18th.
#3 Only Flats or Track Shoes with 1/4" or less spikes may be used.

4 events only (including relays) 13-14; 15-16; 17-18
8 & Under Primary 9 (Sub-Bantam) 10 (Bantam) 1989 or After
11 (Sub-Midget) 12 (Midget) 1987-1988
13-14 (Junior) 1985-1986
15-16 (Intermediate) 1983-1984
and 17-18 (Seniors) 1981-1982 or still 18 on 8/8/99
Relays:
1. Write the name of all four relay team members on the back of the entry form as well as up to 4 alternates and the name of the relay team.
2. Each Team Member must send in an entry form, entry fee, and a copy of his/her birth certificate. Please mail entries of all four team members and alternates in the same envelope.
3. A relay counts as one of the four events.
4. All members of the relay team must be in the same age group and the same gender.
5. All team members must wear identical tops.
