Instructions:

  1. List all competitors in the name column, as they appear on your WVSSAC eligibility form.
  2. Limit 4 individuals in sprints and hurdles.  On meet day you can scratch or substitute.
  3. When entering a competitor in an event, please list the time, height, or distance for that competitor.  Coaches may be asked to substantiate an entered time, so have supporting documents for proof.
  4. Relay teams:  If you wish a competitor to be entered in a relay, place an “X” in the appropriate relay column.  Enter your relay times at the bottom of the form.
  5. Please print or write clearly.  Use a ballpoint pen.  Do not use a felt tip pen.
