2004 Junior Olympic Entry Form and Relay Declaration Forms Available
The individual entry and relay declaration forms for the 2004 Junior Olympic Track & Field program are available at www.usatf.org/events/2004/USATFJuniorOlympicTFChampionships/entry/. They are designed as “pass through” forms that can be utilized as the entry document for all levels of competition (except preliminary meets) in the Junior Olympic program. The athlete submits the form to enter the Association Championships, as the athlete qualifies, the same form is passed along (with payment) to the next level of competition.

USATF Athletes (13-18 yrs.) Invited To Apply for USOC “FLAME” Program
Finding Leaders Among Minorities Everywhere (F.L.A.M.E.) is a fun, interactive five-day program that will fly selected participants to the headquarters of the United States Olympic Committee (USOC), expose them to Olympic athletes and introduce them to other outstanding student leaders from around the U.S.
F.L.A.M.E. 2004 is sure to be outstanding as America's athletes will be at the U.S. Olympic Complex in full force preparing for the 2004 Olympic Games. If you are a minority student between the ages of 13 to 18, that demonstrates strong academic and community involvement, or know someone who should apply, download the application today at www.usoc.org.
Application materials must be postmarked by April 23, 2004.
The 2004 F.L.A.M.E. Program will be July 7-11 at the U.S. Olympic Complex in Colorado Springs, Colo. The itinerary will be available in June 2004.
