Coaches and Parents,

The following forms need to be distributed to student athletes so that parents can sign the consent/waiver form. Clip and make copies for the students coming and make sure that parents have signed them. From an organizational standpoint, you should collect all consent/waiver forms and bring them together if your teams are attending together.

Clip the top form below and fax it back to us so that we can plan meals for registrants. We have the McDonough Center to ourselves so we can house as many athletes and coaches that you want to bring. Please help us create a well-planned clinic. We have some other clinicians coming onboard as we speak, so plan on a productive and informative clinic. Come dressed, but make sure the consent/waiver forms are signed in order to participate.
