4 x 1.5mile Relay Entry Form

Coaches, when filling out this form it is very important that you enter the athletes in the order that they are running on each relay in order to get correct splits for your athletes

For those not familiar with excel, to enter an athlete click in the box and just type in their name(First name Last Name). Incomplete relays are accepted.

Coaches who are entering both boys and girls relays, please use the top form for one gender and the bottom form for the other gender
